What is Partners for Healthy Children (PHC)?
Partners for Healthy Children is a health insurance program for children. It gives free health care to children in families with low income.

How do I know if I qualify?
Qualifying for Partners for Healthy Children is based on the number of people in your family and your household income. There is an income chart located on the application.

How do I apply for Partners for Healthy Children?
PHC applications are available at Department of Health and Environmental Control clinics, local Department of Health and Human Services offices, and other locations.

What do I need to send in with my PHC application?
You will need to send proof of your household income from the last four weeks, and documentation of payment for any child care arrangements (if any).

What are some documents that you will accept as proof of income?
Earned (income from work)

Wages -

  • Salaries - Pay stubs or a signed statement from employer
  • Irregular (Odd Jobs) - Statement from the people for whom you worked
  • Self Employment - Most recent federal income tax return, including a copy of all schedules

Unearned - (Other Kinds of Income)

  • Educational Grants/Loans/Scholarships:  Loan Agreement, receipt/statement from person making the loan, or award letters
  • Child Support/Alimony:  Documentation from the family court or written statement from paying parent
  • Lump Sum Payment:  Call for details
  • Social Security, SSI, and Railroad Retirement Benefits:  Award Letter, copy of check
  • Unemployment Benefits:  Award letter, copy of check
  • Veterans (VA) Benefits:  Award letter
  • Worker's Compensation:  Attorney's statement, claims adjuster statement, copy of check, or Industrial, Commission award letter

    How many pay stubs do I have to submit with the application?
    You must submit enough to cover the four weeks prior to your application. Copies are acceptable.

    How long does it take for my PHC application to be approved?
    Applications are processed within 30 days of receipt. Certain cases, however, take longer to process due to special circumstances.

    How will I know who in my family is eligible?
    You will not be eligible unless you are under the age of 19. You will get a permanent card from the Partners for Health (Medicaid) program for each eligible child.  Keep this card, even if your coverage ends because the card can be reactivated, if you re-apply.

    Are income limits determined by gross or net income?
    Gross income amounts are used to determine approval.

    Does child support count as income?
    Yes

    If I don't have any income, what should I do?
    Enter $0.00 where the application asks for your income.

    I have other insurance. Can my child still qualify for Partners for Healthy Children?
    Yes, Medicaid would be your secondary insurance.

    I have medical bills that I owe and have been approved for PHC. What do I do to get these bills paid?
    On the PHC application, indicate that your children have received services within the last three months. When your application is approved, their eligibility will cover the three months prior to making the application. Take your Medicaid card to the office where the medical services were provided. The provider will then be able to bill Medicaid for your unpaid medical bills for dates indicated on your Medicaid card. 

    My child's Medicaid coverage was terminated when he turned 1 year old, what can I do?
    Complete an application for Partners for Healthy Children.

    My child already has Medicaid. Can I complete an application for Partners for Healthy Children?
    Partners for Healthy Children and Medicaid are the same thing. No new application is needed.

    Are dental services covered under Partners for Healthy Children?
    Yes, you will be able to obtain dental services for your children under the Partners for Healthy Children program.

    Is the Partners for Healthy Children program for adults?
    No, it is only for persons under the age of 19, Adults may be covered under a different program, if eligible.
